If you are interested in making an appointment, please read over all of the information on this page and then call our office at 704-366-9930.

**Please let us know if you will need assistance with the stairs to our second floor office.  NO elevator is available.  Visits to another office location can be made.** 

For an initial appointment, you need to bring with you the following:
1. Proper photo identification (i.e. drivers license or school ID if under the age of 16)
2. Insurance card (primary and secondary if applicable)
3. Authorization number for your visit (call the Mental Health side of your insurance to obtain this number)

         **NOTE:  Dr. Ewert is a neuropsychologist, medical insurance will not cover your visit.

4.  Proper co-pay or payment for the appointment:

• We accept Visa and Mastercard, cash, or check.
• Payment is expected at time of service unless proof of coverage through your insurance is shown.
